Building Bridges Health Care project serves families affected by mother's addiction. |
Outcomes
- Developed portable model for continuing care/disease management for mothers (pregnant or parenting) who have been released from inpatient residential treatment program. Identified and documented barriers experienced by released patients who require access to health care and social support services.
Lives Affected
Targeted 20 families transitioning from inpatient program.
Cost Effectiveness
Leveraged $260,000 in cash match and $2.5 million in-kind match. Project is still in early stages where documented findings are not yet forthcoming.
